Confidentiality of Information
All patient health care information at St. Francis Medical Center is confidential and protected by State and Federal laws and HIPAA regulations.

How Do I Obtain My Medical Records?
A patient, or his/her legal representative, may inspect and/or obtain a copy of their medical record, or have copies of medical records sent to another facility.

St. Francis Medical Center requires a completed, signed and dated Authorization for Release of Health Information form before releasing documents to anyone, including the patient. Because a signature is required to release your information, we are not able to process requests received by telephone or email. When you pick up your records in person, we will ask for photo identification. There may be a fee for providing copies of the medical record. Electronic Request for Records
St. Francis Medical Center now offers an online records request tool that allows you to securely access your health records. We are dedicated to ensuring the privacy of protected health information, and only allow the following individuals to request patient records:

  • Patients requesting their own records
  • Parents of minor patients requesting records
  • Caregivers acting on behalf of a patient (Ex: Power of Attorney)

Our online tool verifies the requester’s identify by requiring a photo of their driver license, which can be taken on a webcam or smartphone. There is no additional charge to use this service. We recommend using Chrome, Safari or Firefox for the best user experience when requesting records through our tool. If you are a current patient using MyChart, you can also request records through your MyChart account.
