We invest in three key areas, based on the Priority Areas identified in the St. Francis Medical Center's 2021 Assessment. These Priority Areas are:
Consideration of Funding
After considering how strongly a proposal aligns with our Community Health Needs Assessment Priority Areas and with St. Francis Medical Center's Core Values, we evaluate every request according to the following criteria:
The SFMC Mission & People Sponsorship & Donation Committee meets monthly to review requests. Applications must be submitted 90 days prior to the event to be considered.
Before filling out the application below, Download, fill, and save this Ownership Acknowledgement Form. It must be submitted as part of the application below.
Organizations and/or programs that discriminate based on race, color, religion (creed), gender, gender expression, age, national origin (ancestry), disability, marital status, sexual orientation, or military status will not be granted funding.

St. Francis Medical Center's Mission
Inspired by the vision of St. Francis of Assisi and in the tradition of the Roman Catholic Church, we extend the healing ministry of Jesus Christ to God's people, especially those most in need. We call forth all who serve in this healthcare ministry to share their gifts and talents to create a Spirit of Healing for all those entrusted to our care.
